I just came across this post and wondered if you managed to get the decoding working. You obviously did a lot of work figuring out the protocol, so did you take it further than that?

Sounds like you've exhausted all the possibilities then. It's still possible there's a problem with your setup and the reader itself is OK, but it's probably about time to send it to P1D and see what they say. Found a stash of P1D readers in a box :-) I've tested with both a USB-serial converter and also an EtherTen wired up as Angus suggested, and both methods worked. The Priority1 Design site shows this one: http://www.priority1design.com.au/rfidread-mrw.pdf According to that spec sheet that particular reader supports EM4100, so perhaps you have a different model.
